How to process certification/verification of OFW records

For overseas Filipino workers who needs certification or verification of their records, here are the documents needed and the steps to follow.

Documents

If the certification is requested by the OFW himself, he/she should prepare the following:

  1. Accomplished verification request form

  2. ID with picture (passport, driver’s license, PRC card, NBI clearance, company ID, etc.)


If the certification is to be processed by a family member or relatives, bring the following:

  1. Accomplished verification request form

  2. Proof of relationship (original or authenticated or certified true copy)

Spouse - marriage contract

Children - birth certificate of the requesting party

Siblings - birth certificate of the requesting party and birth certificate of the OFW

Parents - birth certificate of the OFW

  1. ID with picture (passport, driver’s license, PRC card, NBI clearance, company ID, etc.)


PROCEDURE

  1. Get a service number and verification request form from the guard on duty at the Central Records Division (CRD) on the 6th floor of POEA Building in Mandaluyong City.

  2. Fill out the verification request form and attach the requirements. Then wait for your number to be called.

  3. Once your number is called, proceed to the service window to submit the accomplished form and requirements for evaluation. If your documents are complete and in order, the requested records will be verified and printed. However, if it is incomplete, a verification status form will be given stating the documents the needed to be complied with.

  4. Wait for your name to be called at the releasing window.

  5. Proceed to the releasing window when your name is called. A duly certified printout of information sheet or certification of no record will be released to you.

*If the record you requested needs manual retrieval of contracts and/or OFW information sheets, you will be given a verification status form which will bear the date you have to return for the requested record.

  1. Sign in on the releasing logbook as proof of receipt of the record.



Processing of requested records will be fast if the requirements are complete and no need to produce other documents.
